New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent council tax, utility bill or bank statement. We also require one form of photographic ID (a valid passport). You will need to bring the original of these to the surgery in order to register.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
We invite all new patients for a New Patient Health Check or NHS Health Check with a member of the healthcare team to ensure that any required tests are up to date. Please contact reception for further information.
Medical treatment is available from the date of registration.

The large area is the inner practice area. Patients living in this area may register with our surgery.
The section south of Lower Mortlake Road/Lower Richmond Road (down to the railway) is the outer practice area. Only existing patients who move to this outer practice area may remain registered with our surgery.
